Postoperative paralytic ileus after a high dose of fentanyl: A case report

Abstract

|
Background : Fentanyl is a strong analgesic which is one of the most important drugs used postoperatively for pain control. However, high doses of fentanyl have been associated with postoperative complications, including paralytic ileus. In this report, we present our experiences with a case of severe paralytic ileus after fentanyl administration for postoperative pain management.
Case : A 61-year-old man underwent open distal gastrectomy and was administered with a total of 400 ¥ìg of fentanyl in the post-anesthesia care unit with the intent to maintain the pain intensity at a visual analogue scale score < 3 at rest or < 5 during wound compression, according to a clinical study protocol. He was diagnosed with paralytic ileus on postoperative day 4. He received conservative treatment and was discharged without sequelae on postoperative day 10.
Conclusion : This case encourages anesthesiologists to be aware and implement strategies to reduce opioid doses when managing optimal pain control for enhanced recovery after surgery and overall outcomes.
